In the bowl of a stand mixer fitted with the paddle attachment or a large mixing bowl with an electric hand mixer, blend the softened 8 ounce cream cheese until smooth.
Add the ½ cup sugar, ½ teaspoon vanilla or almond extract and ½ teaspoon coarse kosher salt, blending until mixed.
Spread the caramel apple mixture into a small casserole dish.
Top with the 1 ½ cups thick caramel sauce and ½ cup toffee bits. Add the ¼ cup pecans or walnuts chopped and toasted and ¼ cup mini chocolate chips if desired.
Cover and chill until ready to serve. Serve with the Apple slices.
